MX-4501N repeated holes in primary transfer belt?

JUST NEED to check in case there is something we may be missing on belts getting holes in them.

First time, appeared customer got foreign object into paper path as ding in fuser plus matching hole in belt and then torn belt scratched black drum. Thoroughly cleaned everywhere, cleaned other image units etc. and checked .

Replaced all the above components. Was good for about 8K black and 4K color.
hole in transfer belt about 3" in from rear. Customer swears they haven't in anyway dropped any thing onto by-pass, nor "dug-out any jams with an object. Our sense is that yes, this is THE ONLY was an OEM belt with so little use would get a tear like that.

Are we missing something re the belts? On other brands, when they fail as a unit it usually seems to be from the edge.

i would take a close look at the cleaning unit. make sure its not seizing up, and the cleaning blade is in good shape. i can't see anything else in the machine damaging the belt

Thanks! Agree with you. Blade /cleaning unit OK - relatively new and not binding. IT just must be user screw ups. The first time it was a foreign object they dropped onto the bypass with labels. This time appears someone used something like a pen or pointed object to pry out a jam. Then it s like oh we wouldn't do that so it must be the machine.
So even though I was already in agree with what you mentioned, I thought well perhaps, there is some odd thing I am missing. Even though have thoroughly checked entire paper path as well as transfer chamber interior and the belt itself - cleaning unit etc.
