has anyone had any experience of setting up a template for a fac header sheet using the fax driver?

It's been years but I doubt they have changed it very much. It's not very user friendly at all. You can use the basic information entries and choose from some really simplistic cover sheets or generate one of your own from the same page that you choose the cover sheets from. Choose edit and make your own cover sheet. After you make the cover sheet you can save it as one of your templates.
Basic steps are;
Click print and choose the fax driver
click print, the fax information window will pop up
enter your fax number
click next
this window is where you enter your cover sheet info if you want or create a new one by clicking edit after choosing the cover sheet bullet

PS Supposedly you can import cover sheets but I have never had any success with this feature.
