Mx-5050 c4-20 code after transfer belt shredded

We have a newly installed mx-5050 after 2 weeks of install the transer belt shredded. The tech ordered a new transfer assy installed it and the machine went to a c4-20. We verified that all drums and dv units moved properly as well as transfer unit. We tried replacing the tw pcb.but it still throws the code. Any suggestions would be great. Thank you

UPDATE We found the problem was the brand new belt unit was missing a bearing under the belt for the bias roller right out of box. and the reason the first one shredded after 2000 copies was bulletin tt-102016 for changing sim 55-1 softswitch 12 data 4 to 1 so yellow toner is put on the transfer belt so blade doesn't stick to belt. or at least lets hope that's why the first one shredded..
