Toshiba 3551 - What is it?

Once again I come forth with a question about the nightmare 3511, but lets see if anyone has seen this before.

When copying at 65% or lower (I imagine) the corner of the print has a nice black image on it that should not be there. I also did a scan at full size and the image is in color. Cleaned the optics and glass in the upper and cleaned the laser unit glass. The image would be on the right corner of the glass towards the operator panel. If I put a blank piece of paper over that are or something to copy this strange image still shows on the copy. I even swapped the glass with another unit.

One of the following is usually what I see with this error.

one of the carriages is out of alignment
one of the mirrors is loose
the teflon tape the carriage rides on is not right
one of the carriages is hitting something
the glass is not completely flush
the doc feeder is not down all the way.
And Star Trek was just a tv show...yeah right!

I'm thinking it is something on the carrier as I just replaced the lamp and three copies were fine at 65% Switched to 50% and heard a clack and the image was back. Went back to 65% and the image is still there. Looks like a reflection of the lamp light. Time to get out of here, have to look at it more tomorrow.

Power OFF > hold down 9 and START > Power ON (still holding 9 and Start) > (List Mode should be displayed) > Enter 103 Start (this will print out the P.M. Report). Also 106 Start will print out the Error Codes.
