Xerox Phaser 5550 not pulling from correct tray

Hi.

I'm having a rough time with an Xerox Phaser 5550DT printer. It randomly will pull from the incorrect tray. Cant duplicate on demand, but I've seen it happen with my own eyes.

First the tray setup:
Tray 2: Plain Paper, Letter LEF
Tray 3: Letterhead, Letter LEF
Tray 4: Plain Paper, Legal
Tray 5: Plain Paper, Legal
Tray sequence is Off
Tray 2-5 Mode is Dynamic

Customer has the driver loaded on a server and all the users are mapped thru the server. Currently using PCL6 driver, but have tried PS and PCL5 drivers with same results.

Here is the scenario. User will print (screen shot function from AS400). Properties specifies to auto select tray. It will print on plain paper. Eventually print again this time it will pull from letter head. unfortunately this is random and cannot be done on command, but eventually is will pull from wrong tray.

I have replaced the engine board and reset the NVM, (both in the control panel and in diagnostics.) The latest NVM reset in diagnostics (Done in February) was the best, it was only pulling from the incorrect tray maybe once every 2 weeks, but now in late April, it is happening with much more frequency.

On top of it all, sometimes during a large print job from tray2, when the tray runs out of paper, it will move on to tray3.

Any one else seen any thing similar?

The app is specifying plain paper and the driver is set to auto select.
