I think it is a jam in the hole punch area.

012-909 , Punch Out sensor static jam. (PL 17.117) Test the sensor in service mode input checks 012-105.

If no paper is present at this sensor, possible to be caused by sensor wires being crimped on to connector too tight breaking the wire at the connector and also tie wraps are too tight causing breaking of wires.
