if the exit sensor arm is not moving smooth that's the cause of your problems, if a sensor arm (or actuator)is not returning to its original position timely that will cause multiple problems (most of them jams). go and clean the sensor arm from debris, dust, etc and put a single drop of oil on the shaft for it to move smoothly.+oh yes, and air-clean the sensor itself.
and that's it !
