Yeap we can ask the less obvious questions and have them check this or "I saw it do this once because" sort of stuff. But at least we won't be doing stuff like whoever it was that made this error

Your are right; of course, BC (Master of the Obvious) would have caught this first thing without needing to respond. But we don't always get such a photo clue to what might be the problem.
Errors like this are because we aren't paying attention to the task at hand.
