No jamming in the duplex at all?
Have you checked the jam codes for the history?
Because you say it does it when printing AND copying we can rule out the DADF...even though the issue is out in both the scanning direction AND the sub-scanning direction.
The duplex rollers in the iR5000 are notorious for flat spotting causing jams,skewing and 2 sided registration issues....check them all by turning them by hand and feeling for "bumps" as you turn them.
Check the duplex clutches and if all else fails swap in a duplex driver pcb from a working machine.
Another outside possiblity is the fuser.
As the sheet re-enters the fuser for the 2nd side it might be a speed issue...check fuser life.

Check both the Flag, Regist Sensor (FB5-3335-000) and the Motor, Stepping, DC24V (FH6-1253-000) in the duplex assembly.
The sensor arm that sets the front/back registration gets little cuts in it from the paper passing over it all the time, sometimes cutting all the way through.
The other culprit is the motor that drives the sensor arm, the little gear attached to the motor often has a fracture in it stopping the arm from moving intermittantly.
Unrelated but it probably wouldn't hurt to check the other sensors in the duplex assembly for wear and abrasion while your at it as I've had to replace those around the same time also. The other common issue I've had with the duplex assembly is the bearings on the registration roller wearing out. Roller, Steel (FB5-3322-000), Ball Bearing, NMB DDLF-1060ZZH (XG9-0192-000) & Washer (FS5-6488-000). The bearing gets ground out which in turn cuts into the steel roller and causes it to flop about loosely, if it has more than 1-2mm of side to side movement in it I'd replace that as well.
