Almost sounds like a memory issue- have you swapped that out yet?

Another thing I should have thought of besides what mascan42 asked- what happens when you scan to the hard drive, i.e. user box?

            Update the firmware to R6.

Afterwhich, I usually do a data clear operation. Backup your one touch and copy track data before you do this. Page Scope Data Admin is recommended. Print out all your network settings. Then do the firmware, and follow that up with a data clear.

Your hard disk will be disabled, be sure to re-enable it in tech rep. If you had a fax unit installed, you will also need to re-enable that option in tech rep. Be sure to initialize the fax board in tech rep as well. Upload user data with Data Admin. Program network settings.

If these steps do not correct the behavior, check the memory on the MFB, including hard drive. Reseat connections on these.
