How do I get rid of a Prescribe form?

I have a FS-3900 that prints a letter head on word documents. Kyocera Hot line told me to format the flash card. Now I get a flash card error every time I print. I can press go and it will print without the letter head, but I get the error everytime. Kyocera told me to update the firmware but this did not work. Anybody familiar with this?

I formatted the card in the printer and the customer does not want the letter head. I get a flash card error oo5 with or without the card inserted.

Check to see if the print job is using a logical port on Command Center. You may have removed the form, but the logical port is still pointing at where it thinks that it is. You can just delete out the routing information on the logical port. =^..^=
If you'd like a serious answer to your request:
        1) demonstrate that you've read the manual
        2) demonstrate that you made some attempt to fix it.
        3) if you're going to ask about jams include the jam code.
        4) if you're going to ask about an error code include the error code.
        5) You are the person onsite. Only you can make observations.

If you're printing through a windows driver, check the prologue/epilogue section of the driver under printing defaults then advanced tab. Remove anything that might be in there calling the macro.
