Good Lord is that on platen or ADF? Usual Suspects is Lens Block/BICU Assembly

Someone has been leaning on the left front corner of the machine. Lift the left front corner up till the copy clears up. It may take a couple of times to get it back to close to normal.

This is an 816mf. I don't think you can "Lift" the corner up. This is a one piece unit without the scanner just "attached" It someone leaning did this wow.. can that be fixed.

          Try to clean scanner first

Try to clean the whole scanner unit first.if it change something there is much dirt on it at second check the belt from scanner actuating somethimes it gets loosen you can ajust it with 3 screws from the rear side.
          good luck

I think it is one of the mirror frames out of alignment. I saw a similar one when UPS delivered a machine to a customer and they tried to set it up and bent a frame instead of removing the stays at the bottom that hold the frames in place. It wasnt too obvious unless you know what you're looking for.

Almost like they Knocked it on the FLOOR!!! But then again the platen would must likely be broken.

make a skyshot with no paper/lid up and scanner assembly moves straight?

I'm with Vulkor here. Something is definitely crooked. Notice how badly skewed the image is.

Take a close look at the mirror and mirror holder on the left end of the scanner bed. Check that it is exactly lined up (parallel) with the platen and slit glass. I'm gonna guess that the end of the mirror toward the front of the machine is shifted to the right compared to the rear of the same mirror.

You might also want to look at the bottom four corners of the copier to see if any of them are deformed as in being dropped.

Assuming you made this copy with the top of the page against the left scale, take a close look at the rear of all of the mirrors. Perhaps a scanner skid pin is missing or broken causing the out of focus. However, that doesn't entirely explain it as the focus problem seems pretty limited to one area. Can't really tell based on the sample you provided.
