Probably the 2 most likely causes will be the feed clutch or the sensor pits. The pits collect dust and eventually the sensor can't get a proper reading. Make sure you clean them out.

Agree with teebee. When was the last time the feed belt, pick up roller and sep roller were changed? that is always a good place to start

You may take a look at the "skew correction roller" it gives the same symptoms like a bad clutch. What I've seen on other copier ADF's is that the rubber separates from the shaft, this is more likely when the customer uses the doc feeder a lot.
        I hope this helps.

generally I would clean the belt and paper pickup roller if there is tread left and replace the auto reverse roller, if that doesn't work the pits for the sensors is def a good place too look I also have had to replace the photoreflective sensors in the adf cover that detect the paper

Check out the gear collar on the ADF bracket that slides across when drive is provided.
If the collar is dirty or has been oiled by another tech it can cause it to intermittently stick and not freely shift across.
Also don't forget to clean the sensor underneath the document feeder (2x screws to remove the sensor cover).
