All the samples I have seen so far this issue is always at the header. This copier has had a firmware update with in the last 6 months.

Could be a memory problem. Saw this a few years ago on an AF3228.

      Regarding the printing issue....
I've had experiences like that when printing certain PDF files with the RPCS driver, and switching to PCL5 has always solved it. Just wondering if you may be using the RPCS Driver? I ONLY use PCL5 now for all machines.

I've experienced some garbled output on random incoming faxes here and there, and solved that by slowing down the modem. I assumed it was caused by lower quality lines in the area and/or building.
I'm not sure of the exact setting, but it was a bit switch setting (you'll find it in the manual)
