I'd see whether the fault reoccurs, try it from a laptop via a crossover cable, then rip out all traces of the drivers and start from scratch.
It's most likely an application issue though - try a few different applications.
Finally, some of the most up-to-date drivers such as the latest Universal Print driver come with duplex on as default.
Finally finally, has an edited version of the print driver been installed, maybe for pop-ups for using user codes. These can cause some odd faults if a non-edited version is also installed.
I haven't heard back from them since I went there last week. As far as I know its sorted.
