Most of the toner supply problems we've had with most of the B&W range have nearly always been the toner bottle stop cap being stuck or left over from the previous bottle.

I've had the toner add clutch fail on the dev motor pack. It's not replaceable on it's own, it must be replaced as an assembly. If you run the clutch in output mode, you should get a good, solid click out of it. If not, it's had it.
It's 106 miles to Chicago. We've got a full tank of gas, half a pack of cigarettes, it's dark and we're wearing sunglasses.

      Hit it.

Hello Ruben, this happen on mid volumen ( 22 to 45 PPM ) but on 55-75ppm the system is diferent, the toner hopper on the developer unit is full of toner, but the toner feed motor clucth must be activated in order the toner fall on the developer

You can also have the holes in the toner metering plate on the bottom of the toner hopper can become clogged, especially if you are using after market toner.

I don't know if this applies anymore, but higher speed Ricoh machines were built to be used in higher print volume applications. The upshot of this is that if you don't use the machine enough, the toner feed motor doesn't run long enough, and over time the toner becomes depleted causing low density prints.
If this could possibly be the case, you can either run higher print count jobs on the machine or reduce the toner feed time value in service mode.
Or it could just be non-OEM toner. That's somewhat improbable though.
Sorry if this is a waste of time.

When you adding toner via SP mode connect the meter to the motor terminals and see if there is any power going to the motor and trace the wires all the way to the IOB board and test the pins on the connector. Also check the toner sensor on the hopper. I know you said you changed it but if it doesn't make the whistling sound when you connect it it could be the wire leading to it.

double check the dev unit there is a filter on top that you need to swap off old unit if not it will dust the laser glass and light prints...
