Can not log in to MX4141

    [Misc] Can not log in to MX4141

    This machine has User control enabled with it normally defaulting to a B&W account. They have user codes for color. It has worked for months until recently. The problem is this:
    1) Once the system boots up an error message appears "Device Account Mode Failed, Contact your Administrator".
    2) Can log onto the webpage using a User password but access to the menu is very limited due to permissions.
    3) Cannot log into Administrator or System Administrator, "Authentication Failed"
    4) Reset System Admin password with 24-30 (no DSK installed) but that didn't change anything.
    5) Entered Simulations and upgraded firmware to 03.00.R2, didn't change anything.
    6) Cannot enter system settings from display console or webpage.

    It appears the machine is not accepting any passwords.
    My questions are; where are the passwords kept and what authenticates them to enable use of the machine. Are they stored on the SD card and processed by the MFP?
