Make sure that the hinges that attach the ADF to the brackets are tightened and check skew adjustment in service manual. In my manual it is on page 8-6 and 8-7.

We had this issue with an MXM700N. Changed out every board, roller, ccd and CIS before realizing the hinges were not tightened. Never had the issue come back since tightening them.

    Good luck!

In the manual it is when a square is misaligned , in my case it is more like a wave.

Check your exit follower rollers right at the left side of the scanner body. Will get this effect if they are dragging. Haven't seen this much on the MXM350 series but happened with the ARM355 series on a regular basis. Basically the same machine with a few exceptions.

        Good luck.

Adjusting the hinges on the MXM700N fixed the issue that looked exactly like your test copy.
