I have seen this issue across a few series of mfp's, mostly 4515ac's, 5005ac's. The mfp is set so the customer cannot change the paper size. I set tray 1 to plain LT then somehow it gets switched to LG plain. auto inch is turned off. I haven't been able to confirm if someone was given the password that shouldn't have been and is just switching the tray on their own. I highly doubt this as the paper inside is still plain LT and if they know how to switch it to LG they should know to switch it back when they reload. When you switch it to LT it does recognize that change within the 08 codes. I'm thinking firmware re-write even though its current??

I've seen this exact same issue many times. I have a theory, but haven't been able to prove it. I'm wonder if a driver is configured incorrectly and sends the paper to a specific tray, instead of using auto paper size select. Once it arrives, perhaps the job goes to the skipped queue and when you select it to release you are given an option to manually change to tray to the paper size required by the print job? It's a theory, but a tough one to prove.Why does every customer think they're the first one to ever say: "you've been here so much we should get you an office, ha, ha, ha"? -

It sorta reminds me of a situation with the bizhub PRO 950:
In the Konica Minolta print driver, if you click the drop down menu for paper size the first item on the list is 8 1/2" x 11" W. Many endusers falsely concluded that was the standard paper size. Actually, if you browsed down two full screens you might see the very last item: US Letter.
So it only took one enduser to select the 8 1/2" x 11" W paper size, then sent a print job. Naturally, the machine said that it didn't have that size (which it doesn't). The enduser the forces 8 1/2" x 11" W paper size to the tray, following screen prompts. That job prints, but now every other user that was correctly selecting US Letter is screwed, since the paper size doesn't match.
Every single service call I was re-selecting the correct paper tray sizes, often for all the trays, and trying to explain for the zillionth time why the enduser should NOT choose 8 1/2" x 11" W paper size. I want to thank Konica Minolta for placing US Letter, THE most common paper size, dead last on the list. =^..^=If you'd like a serious answer to your request:
1) demonstrate that you've read the manual
2) demonstrate that you made some attempt to fix it.
3) if you're going to ask about jams include the jam code.
4) if you're going to ask about an error code include the error code.
5) You are the person onsite. Only you can make observations.

I've seen this problem happen on an older machine. It happened when the paper tray ran out of paper and the machine was set for auto on tray one, which was the manual feed tray. The machine would pop up a message asking to add paper and press start to continue using tray one, instead of allowing the user to put paper in the empty tray.
So, you could put a stack of paper on the manual feed tray and let the print job finish. Then when you go back and look at the paper sizes set for the paper trays, the one that ran out of paper was always changed to legal instead of letter. The short term workaround was to set a specific paper size on the manual feed tray and after figuring it out, a later firmware update resolved it long term.Comment

These trays are very sensitive and have had a few customers on the phone whilst they are slamming the drawers, jolting the guides and throwing the size sensing off. Ask them what size is displayed on the machine..."yellow triangle" is not a size; it is the cause of the problem.
These models also have the message when you open and close the paper drawer. It says something like "are you sure" but it should say "did you change something". Customers click "No" then they shoot themselves in the foot turning off Auto Inch and setting a static size or they set it to a different type or thickness.
With printing, if it is asking for paper in the bypass, it is ALWAYS 1 of 2 things: the machines paper size/type is not set properly, or the print job size/type sent from the file is not present in the machine. Printing is like plumbing, there are only 2 rules. In printing its paper size and type must match. In plumbing the 2 rules are shit travels downhill and don't chew your fingernails.

I've seen this more than a few times. Hopefully I'm not reviving a dead thread.Situation
1: A few months ago, I was getting a ton of jam notifications at a Dr's office - a 3508a . Go there, wrong paper size in the tray, auto inch set to auto mm. Install a Kyocera desktop printer and see the same jams on the Toshiba, same thing happening. I select to auto inch, correct tray, customer prints - JAMS. Same thing just happened after I reset the trays. Could not test any further as the driver was installed on a server level remotely. They were logging into a terminal server and the company IT department handled it.
Situation 2-99: I've seen auto inch changed to auto mm randomly. Absolutely zero clue or idea why this happens. No remote FW upgrades / downgrades done, etc. Various models from xx08a to xx06ac and everywhere in between. Have not seen that on the xx15ac / xx18a / xx16ac as of yet.If anyone has a solution, I'm all ears.Comment
