5655 random black page when scan to email

Has anyone ever seen one of these machines throw in a random black page in a bunch of scanned pages ?

just randomly inserts a page that looks completely black and it's not part of the packet that was being scanned

You can watch it scanned everything feeds good replaced to feed head assembly just to make sure but it's very odd

Can you clean the optics very well. remove the glass and clean all the mirrors.
