Versant 80, finisher fault 12-188

We're getting a fault code 12-188 when the paper exits part way out the top tray of the finisher. It's a pro finisher (inserter, 3-hole, booklet maker). They did not select 3-hole. The info I got was it is related to a 3-hole error. Any advise will help. They are using it without the finisher, dumping the prints into a box until I figure out what the issue is.

Do you have access to Pro LIbro? There are several Community bulletins for this jam. You did not state anything you tried.

I would assume a V80 has accumulated a ton of paper dust. I'd start with cleaning the hole punch out sensor. Even if you're not punching.
