I'm going to post this here in case it might help someone.
Recently, I was working on a machine that had significant skew on the prints when the original was fed through the document handler (I think it was a WC5855).
The first thing I did was try the DH registration adjustment, but it would fail. One time it was my fault (Machine says: Hey, dummy, the instructions right in front of your face says to load the paper in the DH SEF! Me: D'OH!), but when I loaded the three sheets correctly the adjustment would fail as well.
Turns out the document handler was physically out of alignment with the rest of the machine. Once I took care of that everything else worked.
