[CQ] HP CM6040 has multicolored horizontal bands at top and bottom of prints

Please refer to the attached jpg samples. Sample 005 demonstrates the issue most prominently, followed by sample 001 and 002. In samples 003 and 004, the bands are present, but much more subtle. I included these samples to demonstrate the varying appearance of the bands on the different colored print quality troubleshooting sheets. The bands never completely go away regardless of the background color.
All supplies, statuses listed below, are genuine HP.

    K toner 72%
    K drum 14%
    C toner 81%
    C drum 40%
    M toner 46%
    M drum 41%
    Y toner 91%
    Y drum 40%
    Image transfer kit 83%
    Fuser 74%

What do you guys think is the problem?
    Thanks

        Wild Bill,
I don't know why the images will not display so that they can be seen. The files are there, but...?
Anyway, would it be ok to email you the images directly? They are fine on my end, but I think that attaching them on my post somehow screwed up the images.

Also, I have calibrated to no avail. The print quality troubleshooting pages are what the samples are. I was trying to demonstrate the appearance of the colored bands on the yellow, blue and red backgrounds of these pages.

        Thanks

The samples won't open for me either, but i'm assuming from your post that these bands are all different colors? I would guess the transfer belt is not being cleaned correctly if that is in fact the case.

            ucinn,
If it is the case that the transfer belt is not being cleaned correctly, what is the fix for this?
            Thanks

New transfer belt assembly...I don't think the cleaner is a separate item on these machines.
