Bent corners, happened between drum and fuser. I knew this because the whole image was present on the paper, even under the bent corner.
Turned out loose toner wore down transfer roller gear that was driven by drum gear. When they didn't turn in tandem the transfer roller would catch a corner and bend it. Replaced drum/imaging unit, tested ok.
