I won't give too much information because I don't want to put anyone on blast. Here's the basic setup:
The machine is a bizhub 454e. Customer reported the adf was jamming. The machine is old, and has never been serviced, but it's also far, far from us. So we decided we would get a contractor to go change the pick rollers, the feed roller, and the sep roller, as well and give the adf a good cleaning. He couldn't figure out how to put the assembly back together after he put the new rollers on, so we sent another assembly up there with fresh rollers. He installed it, and shortly after he left, the adf started jamming and multifeeding like crazy, and started making a strange noise. We ended up getting a different company to go out and check it out, and they couldn't figure out what was wrong, but they did send a picture at my request. This is the picture. Can you spot where the contractor royally screwed up?

Be careful who you pay a hefty fee to do a remedial job was the moral of the story for us.
