        It was indeed something on the slit glass. No idea what, but it took three prep pads and some scrubbing to remove. Whatever it was, it was essentially transparent to the naked eye, but was causing distortion nonetheless.

            If you'd like a serious answer to your request:
            1) demonstrate that you've read the manual
            2) demonstrate that you made some attempt to fix it.
            3) if you're going to ask about jams include the jam code.
            4) if you're going to ask about an error code include the error code.
            5) You are the person onsite. Only you can make observations.

                There are multitudes of threads on CTN that describe the use of methylated spirits (alcohol) to clean stubborn optics faults.

                  OK look the thing is I cleaned it with a cloth and a prep pad before posting. So, to my mind, tried and failed. I also ran my finger down it (didn't feel anything stuck), examined the slit glass closely with a flashlight (didn't see anything), visually inspected the optics with the light as well (nothing dirty, cracked or chipped), tried lining up the line to the glass (nothing obvious at that spot)... considering the platen works fine (not the sensor) and print/copy unaffected (not a print issue).

                  I stand by what I said before: there are ways of conveying advice without being an dick and without assuming the worst in people (neither stupid nor lazy nor unwilling to learn).

                  Example: You probably cleaned the slit glass, but clean it again as some inks are invisible to the naked eye but cause weird distortions nonetheless... so keep rubbing the slit glass down with prep pads until the ADF prints come out fine.
