MP 6001 doc. feed tearing originals

One of my tech.s has 2 MP 6001s that are ripping the lead edge of the customers originals, intermittently. He indicated that the tear is in the very center of the original, and that it looks like a tab approximately 1 inch wide. He has replaced the Sep. roller, pick up roller and the feed belt. The biggest problem is that he has not been able to duplicate the problem, but the customer has called for the same problem for over a month now. And the customer has samples. Any ideas? I'm now headed on the almost 2 hour drive to go check it out. Any input would be greatly appreciated.

Check out the pick up roller cam shaft. It has a torrington clutch-it could be slipping. This is the part that the pick up roller assembly is driven by on the rear side. Also check and clean the input sensors.

Take torque limiter with you, the sep roller is not slipping. A293-6399

I've seen this, but I don't think I ever fixed it because I was quitting my job anyways lol. If I can remember correctly I think it was one of those little black mylars bent out of place where the originals make the turn in the feeder. I think I figured I would have to tear apart the feeder to get to it, and didn't feel like doing it so I ignored it lol.
