It can be the drum, over its limits.

        990 B Software performance error
The software attempted to perform an unexpected operation.

        Software defective
        Internal parameter incorrect
        Insufficient working memory
When this SC occurs, the file name, address, and data will be stored in NVRAM. This information can be checked by using SP7-403. Note the above data and the situation in which this SC occurs. Then report the data and conditions to your technical control center.
        991 D Software continuity error
The software attempted to perform and unexpected operation. However, unlike
SC990, the object of the error is continuity of the software.
        Software bug
        Internal parameter incorrect
        Insufficient working memory

These codes are Ricoh's way of saying"Generic Software Error". I agree with Eric and update the firmware.

Drum worn or charge roller contacts dirty-not making good contact.
          Most common for this machine.
Why do they call it common sense?

If it were common, wouldn't everyone have it?

Make sure firmware is up to date, and replace PCU
