how about on the copier itself, are you able to enter a name and password through the control panel?
have you tried from a computer? a different browser? does the function version match the firmware version?

I'm not that hard on clear all data. When someone knows what to do it's not big deal.
I can do it and setup machine with card reader in probably less than half an hour.
I'm not recommending this as routine but instead of doing circles around machine trying this and that for hours and hours often start fresh can help.

Any chance you have authentication enabled? Specifically external auth and single sign on? I think this would remove the requirements for login name and password as the machine is already on the domain and has rights to certain folders based on this login. E
