      OOPSY #1: Back in the old typewriter days I accidently reversed a connector on the power supply of an Adler Royal. It was like I fired off an M80 firecracker in the customer's office when I plugged it in and hit the switch. The capacitors literally exploded.

      OOPSY #2: Accidently dinged a $1,200 wide format drum during install.

        I did that too! The very first time I had to change a drum on one, at a site 90 minutes away from my office, no less. The ding was RIGHT on the edge of the printing area, over half of it wasn't even visible, but of course there had to be the one little mark that WOULD show up consistently.

        Service manager informed me I could use clear nail polish to fill in the ding, worked like a charm. Fortunately one of the ladies on site just so happened to have some, it took approximately two weeks for the polish to settle into the groove and 100% remove all traces of damage, but afterwards I had no problems with it.
        Cthulhu for president! Why settle for the lesser evil?

          Standing in the archive room at the local GP's office chatting to the receptionist about her machine shaking a toner bottle, didn't realize the spring loaded cap that should seal the container had some how become jammed in the open position.... Had to clean medical records with a vacuum...

            For what it is worth, one time I destroyed a new +$1K wide format drum by ordering the wrong cleaning blade. I ordered the drum and blade just before what was probably the last time I had to go to AT while I may have been in the Reserves, and the tech they had take care of this while I was away put the drum in, but as the cleaning blade I ordered didn't fit even a little bit he left the original one in. That cleaning blade was a delightful color of brown.
            Anyway, when I got back one of the first calls I had was for this machine (NOOOO!!!!). The drum was totally wiped out by then. Had to order another new drum and the correct blade. That sucked.
