then fix the document, it must have been created with process/composite black instead of pure black.
thats maybe why it cant escape printing in color.
ive had 100% luck using foxit reader to print mixed pages that they click correctly.
im sorry you are going through this, sometimes black images or text are dropped over a color one on the page so you cant see it on the monitor but its a color underlying layer and the copier sees this and switches to black overprint instead of just pure black.
We know a thing or two because we've seen a thing or two.
      The medication helps though...

I have used CUPS and the PPD's since i first encountered unix based systems. Try installing the correct PPD using CUPS.
Don't know about all the brands but some will only disengage the CMY if a certain amount of consecutive b/w pages are detected.
It will not change to b/w after 5 pages but will if there is more per example. Not sure if that is the case any more.
        Whatever

That is correct. If the RGB values for "grey" areas in the pdf file are not equal (like R=128,G=128,B=128 etc) the printer thinks it is colour. In the PCL driver (Windows only) there is a setting "covert to greyscale" that allows you to set threshold value for RGB differences 1 to 20 to solve this.
