From what I see you don't have the updated firmware...
Looking at the total counter it has been reset (You could have mentioned it first of all) and probably settings have been deleted that have not been restored.

Right now I think the only solution is to get a Post Script 3 SD card specifically for the C3504ex Type M33. ebay has them listed for $42.00.s-l1600.jpg

considering the time wasted in troubleshooting, I consider it a good deal

the page is incomplete, you could have framed it all.
anyway, I made a sp diagnostic report page from a 2004ex that we have here for comparison, from what I can see from the photo, it seems that some firmware pieces are missing (see attachment) and it seems that overall those that are there, are partially updated, this "could" create conflicts between modules with strange effects, suggestion:
update the machine by downloading all the available firmware using an sd card....and let us know

I never have been able to figure out why some many techs think they need to take a crappy picture of a page with their cell phone when they have a full page scanner right there in front of them.

My bad. Didnt cross my mind to use the scanner. Too comfortable with smartphone.
One question, if I use PostScript3 SD Card, I need to leave it there or install the firmware and take it out?

It will need to remain in the upper slot. The Adobe license file is on the SD card and cannot be moved. If there is another option SD card already there you will need to use SD Card Appli Move to move the application on that card to the PS3 card. Once again if has to do with the licensing.

So, it remain like older version where it rely on SD Card instead of emulation/firmware install internally.

                      Got it. Thank you

look, the postscript is already there on the machines, the sd is just a "dongle" (or hardware key) to activate the license, however during the update it is left inside
this is an interesting problem, you have to let us know how you solved it... otherwise I'll stalk you...LOL
