what default password are you using? can be "12345678", "00000000", "9272927292729272". Also try inputting each password 2 times without a space.

No big deal. Just set an encryption passphrase. It's not unusual for that to clear/reset when you've formatted the HDD. It's in the Admin Settings menu, let's see if I can find specific keystrokes:

In Admin Settings > Security Settings > HDD Lock Password > enter 20 digit passphrase > Ok > re-enter passphrase > power MFP Off/On.

It doesn't really matter what your specific passphrase is, as long as you can type it exactly the same on two separate lines. Essentially Konicas encrypt the data on the HDD with a 20 digit password to make it unrecoverable. =^..^=
If you'd like a serious answer to your request:
          1) demonstrate that you've read the manual
          2) demonstrate that you made some attempt to fix it.
          3) if you're going to ask about jams include the jam code.
          4) if you're going to ask about an error code include the error code.
          5) You are the person onsite. Only you can make observations.

Power Off/On. Three consecutive wrong tries locks the machine. =^..^=

firmware should not have done this.
did you try and swap boards or something?
We know a thing or two because we've seen a thing or two.
                      The medication helps though...

you may need to use the reset tool now. there is a bulletin where to enter part of the serial and code to try and reset.
Use the numbers in the serial twice followed by 8 zeroes. An example for the bizhub C360 serial number is A0ED012020866. (Do not use the item number A0ED012portion.)
The unlock password using the above example is 02086602086600000000.
